Patient Care Manager-Wise, VA

Description:

The Patient Care Manager will be responsible for managing the services of a multidisciplinary team according to the patients' needs and the physicians' orders. The Patient Care Manager will be available to the patients as well as to the field staff to answer questions and assist with problem solving so that the patient's needs will be met in the most appropriate and timely manner. The Patient Care Manager improves quality of patient care by coaching and intervening to facilitate patient progression through the plan of care developed by the Outcomes Manager. Works with a high degree of independence to ensure optimal, clinical, financial, and satisfaction outcomes are achieved in a timely and efficient manner. Serves as a catalyst to challenge current practices and streamline delivery of services.

- Responsible for selection and/or retention of qualified staff
- Set department performance goals, monitors results, and creates opportunities for continuous improvement.
- Create an "expectation of excellence", and improve staff performance, development, and moral, through fair and accurate performance evaluations, daily coaching and mentoring, and/or appropriate corrective action as needed, to reward top performers and improve or replace lower performers.
Completes fair and accurate staff performance evaluations.
Manages department merit increases within Company merit budget.
- Assists with preparation of Medicare, Medicaid, JCAHO, and QA audits.
- Identifies variances in standardized care and care processes to identify areas for improved performance related to patient care and the organization.
- Acts as a positive role model or preceptor for team members new to home care.
- Serves as a resource in assisting the field staff with problem solving in order to provide the most appropriate care management for the patient, serves as a clinical leader.
- Acts as a resource and has a working knowledge of Medicare, Medicaid and private insurance guidelines related to admission/service criteria and its proper clinical documentation.
- Assists with the patient visits in emergencies or acute staffing shortages.

Qualifications:

Must possess a degree in nursing (RN), physical therapy, speech/language pathology or occupational therapy with a valid and current VA license.  Must have  adult assessment skills.

 Requires three years of recent clinical experience, one year of community/home health experience and supervisory experience.
